Weeks into carfilzomib, the microvascular endothelium — often already primed by a silent complement-regulator gene deletion — tips into thrombotic microangiopathy: schistocytes and falling platelets, an acute kidney injury the drug set in motion, and an ADAMTS13 that comes back normal because this was never TTP.

Teaching case · illustrative composite, not a real patient

A 64-year-old man with relapsed multiple myeloma, three weeks into carfilzomib–lenalidomide–dexamethasone at the 56 mg/m² dose, is admitted with fatigue, a blood pressure of 176/102, and dark urine. Labs show hemoglobin 7.6 g/dL, platelets 22 ×10⁹/L, LDH 690 U/L, undetectable haptoglobin, and schistocytes on the smear; creatinine has risen from 1.0 to 3.1 mg/dL with new proteinuria.

The microangiopathic hemolysis with thrombocytopenia and acute kidney injury raises TTP, and empiric plasma exchange is begun — but ADAMTS13 activity returns normal, excluding TTP, and a work-up for Shiga-toxin infection is negative. Recognizing drug-induced TMA, the team stops carfilzomib. Because the kidney injury is severe and dialysis-requiring, terminal complement blockade with eculizumab is added; a complement work-up later returns a homozygous CFHR3–CFHR1 deletion. Over several weeks the hemolysis abates and the platelet count recovers, and he is eventually able to come off dialysis — though his baseline creatinine does not fully return.

Teaching point — Proteasome-inhibitor TMA — overwhelmingly a carfilzomib complication — presents weeks into therapy as microangiopathic hemolysis, thrombocytopenia, and acute kidney injury that mimics TTP. The distinguishing test is a NORMAL ADAMTS13, which excludes TTP and redirects management away from indefinite plasma exchange. The cornerstone is stopping the drug; many cases carry an underlying complement alternative-pathway defect, and terminal complement (anti-C5) blockade is the rescue for severe, dialysis-requiring, or plasma-exchange–refractory disease.

    Morita and colleagues report thrombotic microangiopathy after bortezomib (with dexamethasone) in a patient with multiple myeloma.

    An early historical anchor for proteasome-inhibitor TMA — predating carfilzomib's approval and hinting that the class, not one drug, could injure the microvasculature.

    Hobeika, Self, and Velez publish a kidney-biopsy–confirmed case of carfilzomib-associated renal thrombotic microangiopathy with concurrent podocytopathy.

    One of the first biopsy-proven reports tying the newly approved carfilzomib to intrarenal TMA, with proteinuria and hypertension improving about eight weeks after the drug was stopped.

    Stop the proteasome inhibitor — the cornerstone

    Discontinuation is the essential first step; hematologic TMA resolves in the large majority (about 94% in the largest series), though the acute kidney injury frequently leaves residual impairment, so renal recovery lags and may be incomplete.

    Check ADAMTS13 to exclude TTP before committing to plasma exchange

    ADAMTS13 activity is normal in most proteasome-inhibitor TMA, distinguishing it from TTP; a normal result argues against the ongoing plasma exchange that TTP requires and refocuses care on drug withdrawal and complement-directed options.

    In the largest series, plasma exchange, corticosteroids, and even eculizumab showed no significant impact on outcome once the drug was stopped — underscoring that timely proteasome-inhibitor withdrawal, not reflexive plasma exchange, is what drives recovery.

    If rechallenge is needed, use a reduced dose and monitor closely

    Because the injury is dose-related, a reduced-dose carfilzomib rechallenge has been tolerated without TMA recurrence while preserving disease control — an option when the proteasome inhibitor remains the best anti-myeloma choice, undertaken with close hematologic and renal monitoring.
